Field Experience Request Form

The online Field Experience Request Form, http://cehd.gmu.edu/endorse/ferf/ was developed using HTML, PHP, MySQL, Javasvript, Ajax and Access. This online form automated a paper process and is used by hundreds of students seeking internships each semester. The courses in the dropdown are automatically pulled from the MySQL database with a snippet written in PHP and Ajax. Clicking on the radio buttons will initiate Javascript statements requiring the user to enter relevant data that is specific to that selection. User interfaces were created that will allow the administrator to change the comments at the top of the page and to turn off the form when the deadline has passed. Configured ODBC to directly link by IP, administrator’s computer to the database so that they could update tables using Microsoft Access.
