The Instructor and Course Appraisal forms which were completed for this class included one that I believe was completed incorrectly. The student responded "1" or "Poor" for each of the six questions on the form. However, the answers to the questions on the bottom of the form were:

A. What has your teacher done especially well in this course?

"Very, very supportive, positive, and patient"

B. How might the course be improved?

"Possibly longer for slowpokes like me"

In light of the fact that these answers were generally positive, I have chosen NOT to include the "Poor" responses in this matrix. The average responses for this class were calculated without this form.
